To better assist our Customers we have provided some documents that can be downloaded to your home computer to complete at your leisure.

The Application document:  download, complete, then fax to 480-981-7002.   In order to expedite the application, you may pay online through the Pay Now button.  The Application Fee is $40.00 for each  person intending to reside at the property address.
